Putnam Hospital Center (PHC) is a 164-bed not-for-profit acute care hospital offering the highest quality medical, surgical, psychiatric, pediatric and obstetrical/gynecological care, as well as 24-hour emergency services and home health care. The hospital, which affiliated with Health Quest in May 2001, has 110 medical/surgical beds, a 10-bed intensive care unit, a 10-bed pediatric unit, a 14-bed maternity unit and a 20-bed adult psychiatric unit; a full range of inpatient and outpatient services are offered.
Located on Stoneleigh Ave in Carmel, New York, PHC serves a growing population of more than 150,000 men, women and children who live and work in Putnam, northern Westchester and southern Dutchess counties; it is Putnam County's largest employer with a staff of over 300 highly qualified physicians, surgeons and allied health professionals and approximately 1000 employees.

Nurse Manager
We recommend that immediate concerns by you or your family should be explained to the Nurse Manager on your floor. He/she has many contacts and resources to meet your requests and can find answers to any questions you may have.
Case Manager
Case managers are also available to help you resolve any concerns. Each floor is assigned a designated Case Manager, who coordinates patient care and also provides discharge planning. A Case Manager can be contacted by letting your nurse know that you wish to speak with the Case Manager.
Patient Advocate
If you do not feel comfortable in contacting the Nurse Manager, another resource we suggest is the Putnam Hospital Center Patient Advocate, who can be reached at (845) 279-5711, Ext. 3008. The Patient Advocate is available for patients and family members and will assist in addressing any questions or concerns.
